COVID-19 VACCINATION POLICY

As you know, ChildRoots is deeply committed to the health and safety of the children entrusted to our care as well as our team members and community during this pandemic. After careful deliberation, ChildRoots is adopting a mandatory COVID vaccination policy, which will be in full effect by August 8, 2021. This means, all ChildRoots employees and parents and eligible children will need to have current COVID vaccinations.

As a pro-vaccination community, we feel strongly that ensuring that those who are able to get vaccinated for COVID do so, is a crucial step in illness prevention and transmission- and ensuring the safety of those around us. 

Proof of full vaccination is required for all employees, parents and eligible children. Currently, complete COVID vaccination means, employees, parents and eligible children must receive their two-dose J & J a three-dose series or by Moderna/Pfizer. Any vaccine authorized for use in the United States is acceptable.

This requirement applies equally to those who have previously been diagnosed with COVID-19.

VERIFICATION REQUIREMENTS

No later than March 1, 2022, employees, parents and eligible children will need to provide proof of fully vaccinated status.  Proof may be provided by producing the CDC vaccination card, or a photo copy/digital picture of your vaccination card, to Sonya Rheingold or a Director for verification. 

Whichever format is provided, must include the following information with sufficient legibility for the supervisor to verify it:

the employee, parent or child’s full name, date of birth, type of COVID-19 vaccination given, date or dates given, and the name/location of the health care provider or site where the vaccine was administered. ChildRoots will record vaccination status and destroy any physical records received.

EXEMPTIONS

Employees, parents and eligible children may request an exemption from the vaccination requirements for medical or non-medical (e.g. religious) reasons, including for reasons provided in Oregon and federal law, by the following process:

  • Employees, parents or eligible children who are not able to be vaccinated for a medical reason may seek a medical exemption.  The process to request a medical exemption will require the employee to provide supporting documentation from a healthcare provider. See Human Resources, Sonya Rheingold or a Director for a copy of the Health Care Provider Certification Form that may be used for this purpose.

  • Employees, parents or eligible children may seek a non-medical (e.g. religious) exemption and will be required to provide sufficient information to support the request.

Please contact Sonya Rheingold or a Director if you are seeking an exemption. Exemption requests will be reviewed on a case-by-case basis.

CONSEQUENCES

Employees, parents and eligible children who are not fully vaccinated and who have not been granted an exemption will be subject to suspension of employment or termination of enrollment.

Employees, parents or eligible children with falsified information related to their vaccination status or eligibility for an exemption will be subject to immediate termination of employment or enrollment.

ASSISTANCE

ChildRoots will work to assist employees experiencing barriers to receiving vaccinations, including employees who may not have access to FDA-authorized vaccines. Employees who have received a vaccine not approved for use in the United States should contact Sonya Rheingold for consultation before taking any action to attempt to comply with this policy.

As we have throughout the pandemic, ChildRoots is continuing to monitor all relevant guidance and information, and will adjust this policy if appropriate and necessary in light of changing circumstances and experience.
