STAFF CHANGE FORM

Use this form to share ALL staff changes! Once submitted, entries are shared to this spreadsheet. If corrections need to be made, simply submit the form again. Thank you!

Payroll and HR use the spreadsheet to update Gusto, and the benefits system.

You need only fill out the information relevant to the ‘Type of change’ being made. This also applies to which date information is necessary; not all dates - hire, start, effective, last - need not be filled in, depending on the change.

As always, please be as detailed as possible in the Notes, particularly for atypical changes.
